The Pre-participation Exam - What to expect

• Fill out the pre-participation questionnaire and bring it to your appointment
• Bring in pertinent medical and immunization records
• All kids should be accompanied by a parent
• Intake by medical assistant includes:
  • Height
  • Weight
  • Blood Pressure
  • Pulse rate
  • Vision Testing
• Physician Exam includes:
  • Review of medical history
  • Head-to-toe physical exam
  • Completion of the NIAA Pre-participation Form
• Disposition
  • Cleared for full participation in sports/activities.
  • Limited participation
  • Needs further evaluation (e.g. referral for specialty testing)
