If your child needs a school or sports physical exam, do not hesitate to contact our office. We offer "pre-participation" physical exams for student athletes regardless of your child's insurance status.

The Pre-participation Exam - What to expect
- Fill out the pre-participation questionnaire and bring it to your appointment
- Bring in pertinent medical and immunization records
- All kids should be accompanied by a parent
- Intake by medical assistant includes:
- Height
- Weight
- Blood Pressure
- Pulse rate
- Vision Testing
- Physician Exam includes:
- Review of medical history
- Head-to-toe physical exam
- Completion of the NIAA Pre-participation Form
- Disposition
- Cleared for full participation in sports/activities.
- Limited participation
- Needs further evaluation (e.g. referral for specialty testing)
