Our Membership Fees
There is a $200 fee to reinstate your membership should you decide to discontinue your membership.
10% discount for annual payment.
Additional charges may apply for vaccinations, medical supplies, injectable medications (e.g. Testosterone, B12, Kenalog, antibiotics) bandages, in-house labs, or other necessary tests or supplies between the provider and the patient. All fees will be discussed prior to the administration of treatment with the full consent of the patient or an appropriate representative.
If a service has been provided, membership fees for that period shall be considered substantially earned and non-refundable.

**Due to the increased regulatory burdens, our doctors will NOT prescribe chronic pain medications.
